-If you have an HMO or EPO plan, please make sure to select or switch your primary care physician (PCP) to:

Without this change, your insurance may not cover your visit at Wellness Bay.
-If you have a high-deductible insurance plan and your deductible has not yet been met, a deposit may be required before your visit. Any deposit collected will be applied toward your patient responsibility after your insurance claim is processed.
Appointment Cancellation Policy for New Patients
Due to the high demand for appointments and our commitment to providing timely access to care for everyone in our community, we kindly ask that all cancellations be made at least 24 hours in advance.
If a new patient cancels without a valid emergency reason or without providing at least 24 hours’ notice, we may not be able to reschedule or accept them into our practice in the future.
We truly value your time and ours, and this policy helps us offer appointments to other patients who are waiting to be seen. Thank you for your understanding and support. 💙
